 Homemaker and Personal Care
This
level of service is for the individual who needs
some basic assistance with activities of daily
living, such as transportation, shopping, meal
preparation, housekeeping, avoiding falls,
or medication assistance. Personal care,
such as bathing, dressing, toileting, and personal
hygiene, are also a part of this level of
assistance.
Services
provided by qualified caregivers
and supervised by geriatric care manager.

 Health Maintenance and Assistance
This
level of RN supervised service is for the individual who has chronic
physical conditions and/or significant memory issues. The
involvement of a RN manager and home health aides to
monitor conditions and communicate needs to
families and other medical providers is critical to
coordinate care
from all providers and monitor conditions to avoid
further deterioration. Condition of individual receiving services must be
stable and predictable. |

 Health
Care Management and Assistance
For those who are
recently discharged from a hospital, have significant
medical issues, chronic conditions, or other
specialized care needs, continuing communication with
physicians and other medical providers is key. To
prevent deterioration, manage pain, and promote
healing, the RN Care Manager arranges for services,
communicates with the physicians, and manages care.
She makes regular contact, home visits, and supervises
the nurse delegated home health aide who provides
care.

Higher levels of service include
home care services, as well. Call for more

live-in, and couple care specials.
